OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical in that employees were exposed to fire hazards:   a) On or about October 2, 2025, employees were exposed to fire/explosion hazards while purging newly installed gas water heaters and lines without monitoring the area for gas.

29 CFR  1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: (Construction Reference: 1926.59)   a) On or about October 2, 2025, in the Tankless Water Heater Room, employees had not been trained on the hazards of natural gas while troubleshooting, testing, and purging gas lines connected to Rinnai tankless water heaters.

29 CFR  1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  a) On or about October?2,?2025, in the Tankless Water Heater Room, employees conducted plumbing activities including purging gas lines and igniting natural gas water heaters without the employer ensuring frequent and regular inspections were performed by a competent person.
