29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    On or about October 1, 2025, and at times prior thereto, the employer allowed boards with protruding nails, loose lumber, and debris to remain scattered across multiple areas of the residential construction site, creating a tripping and puncture hazard.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.     On or about October 1, 2025, and at times prior thereto, at the front of the new residential construction site, the employer did not provide a safe means of access for employee working elevated above the first level to install roof trusses being lifted by the lift.  The employees used the cross braces to access the upper work level.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 1, 2025, and at times prior thereto, employees engaged in new residential construction at heights greater than six feet without an adequate fall protection system in place.
