29 CFR 1910.217(c)(2)(i):Every point of operation guard shall meet the following design, construction, application, and adjustment requirements: (a) It shall prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard; a) a) On or about October 1, 2025, the employer failed to ensure that point-of-operation safeguarding was provided and properly used on the Bliss 1500-ton horizontal mechanical power press. Employees were able to enter the press through the interlocked door while the press was in production mode and activate the ejector function, which allowed the press to complete its cycle without adequate guarding to prevent contact with the point of operation. As a result, employees were exposed to amputation hazards.

29 CFR 1910.217(e)(1)(i)(A): Inspection, maintenance, and modification of presses - Inspection and maintenance records. The employer shall establish and follow an inspection program having a general component and a directed component. Under the general component of the inspection program, the employer shall: Conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment; a) On or about October 1, 2025, the employer did not ensure that the required periodic and regular inspection for the Bliss 1500-ton horizontal mechanical power press had been completed to ensure safe operations.

29 CFR 1910.217(f)(2):Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed. a) On or before October 1, 2025, the employer did not ensure that operators were trained and instructed in the safe method of work prior to operating a Bliss 1500-ton horizontal mechanical power press, and did not ensure through adequate supervision that correct operating procedures were followed. Specifically, employees who opened the interlocked doors did not have knowledge that when the machine was stopped mid-cycle, pressing the ejector button would cause the press to complete its cycle, exposing employees to caught-in hazards.
