29 CFR  1926.451(b)(4): Each end of a platform, unless cleated or otherwise restrained by hooks or equivalent means, shall extend over the centerline of its support at least 6 inches (15 cm).  Location: 114 Rose Way Bridgehampton, NY 11932.  On or about October 3rd, 2025- Employees were exposed to falling while removing old plywood on a residential home while working from a ladder-jack scaffold platform. The scaffold plank did not extend at least 6 inches over the centerline of the support.   Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR  1926.451(e)(1): scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access.  Location: 114 Rose Way Bridgehampton, NY 11932.  On or about October 03rd, 2025-A ladder was not provided to access a ladder-jack scaffold platform. Employees were using the ladder supporting the prank to access the working level exposing the employees to fall hazards of approximately 15 feet.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR  1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.  Location: 114 Rose Way Bridgehampton, NY 11932.  On or about October 03rd, 2025- Employees were not protected from falling approximately 15 feet to the ground below while installing removing old siding and plywood working from ladder-jack scaffolding;  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  Location: 114 Rose Way Bridgehampton, NY 11932.  On or about 10/02/2025, employees were accessing the roof of a residential home by using an aluminum extension ladder that was only extended 1ft above the landing.   Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
