29 CFR 1910.119(f)(4): The employer did not implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel. On or about October 4, 2025, in the #2 Solution Nitrate Plant, safe work practices were not implemented for the control of crystallization of ammonium nitrate within a nitric acid line for employees and employees of a contractor that were working on processing equipment.

29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation. On or about October 4, 2025, in the #2 Solution Nitrate Plant, stored energy in the form of ammonium nitrate was permitted to re-accumulate and crystallize in the nitric acid line where verification of isolation was not continued.

On or about October 4, 2025, in the #2 Solution Nitrate Plant, employees performed energy control with procedures that did not specifically outline the steps for placement of lockout devices as energy was isolated from the unit.
