29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about September 23, 2025, and times prior thereto, at this new residential construction site, employees were exposed to fall hazards while performing work at elevated heights and had not received adequate training to recognize and avoid unsafe conditions.  Instance (a): Employee was using a portable A-frame ladder improperly, including using the ladder as a means of access to an elevated platform while running electrical wiring. Instance (b): Employee worked from an elevated work platform that was less than 18 inches in width while running electrical wiring.

29 CFR  1926.50(b): Provisions were not made prior to commencement of the project for prompt medical attention in case of serious injury:  On or about September 23, 2025, and times prior thereto, at this new residential construction site, the employer did not ensure that provisions were made for prompt medical attention. An employee sustained injuries after falling from an elevated height, and emergency medical services were not contacted for approximately one hour to one hour and fifteen minutes following the incident, exposing employees to delayed medical treatment in the event of serious injury.

29 CFR  1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  On or about September 23, 2025, and at times prior thereto, at the interior of a new residential construction site, the employer failed to ensure that each employee was provided a working surface at least 18 inches in width,  and used a 2x4 door frame header as a work platform to run electrical wiring.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about September 23, 2025, and at times prior thereto, at the interior of a new residential construction site, the employer failed to ensure that an employee was protected from falling to a lower level of approximately 8 feet, in that the employee stood on a narrow 2x4 door frame header to perform electrical work.

29 CFR  1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid structure:   On or about September 23, 2025, and at times prior thereto, at the interior of a new residential construction site, the employer failed to ensure that ladders used to access the work location above the door frame extended three feet above the landing.
