29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   On or about October 14, 2025, the employer used a JLG Skytrak 6042 telehandler to support a scaffold platform without ensuring that the entire platform was attached to the fork and allowed the telehandler to be moved horizontally while the platform was occupied by two employees being transported across the parking lot and onto the roof.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about October 14, 2025, the employer did not protect employees working on scaffolds more than 10 feet above a lower level from falling by the use of fall protection. Two employees were on a scaffold platform 3 stories or about 30 feet above the ground level, they were not protected from falling through the use of fall protection.

The employees were not protected by guardrail systems, safety net systems, or personal fall arrest system,  nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501.  Salazar Exteriors Inc. Dba Bull Exteriors was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1640676, citation number 1, item number 1 and was affirmed as a final order on 02/08/2023, with respect to a workplace located at 3617 Collinsville Road, Fairmont City, IL 62201.
