29 CFR 1910.134(a)(1): When effective engineering controls were not feasible or while they were being instituted, appropriate respirators were not used pursuant to the requirements of 29 CFR 1910.134: On or about October 13, 2025 in Plaistow NH. Employees using a gas powered pressure washer inside of Shaw's Supermarket were exposed to carbon monoxide levels of 300ppm while effective controls or respirators were not being used.

29 CFR 1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about October 13, 2025 in Plaistow NH. Employees were exposed to carbon monoxide while operating a gas-powered pressure washer inside an enclosed building. The employer did not provide effective information or training addressing carbon monoxide generation from gas-powered engines, safe operating procedures or ventilation requirements. Employees were not trained to recognize the hazard or the appropriate protective measure to take.
