29 CFR  1910.119(d)(3)(i): Process safety information pertaining to the equipment in the process did not include the elements specified in 29 CFR 1910.119(d)(3)(i)(A) through (H):  On or about October 14, 2025, at the facility located at 701 E Martin St. Coffeyville, Kansas, in area 11, employees engaged in nitrogen fertilizer production activities were exposed to asphyxiant hazards. The employer did not maintain process safety information for the PIC 0615 Bypass Valve. The provided process safety information, required by the standard, was incomplete and did not include all elements identified in the standard. Process safety information pertaining to the equipment in the process for the PIC 0615 Bypass Valve did not include the following: 1. Materials of construction,  2. Design codes and standards employed,  3. Safety systems.  4. The globe valve symbol identified as PIC 0615 bypass valve does not match the P&ID drawings and the PID legends.

29 CFR  1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process.   On or about October 14, 2025, at the facility located at 701 E Martin St. Coffeyville, Kansas, in area 12, employees engaged in nitrogen fertilizer production activities were exposed to asphyxiant hazards. The employer did not identify, evaluate, and manage the hazardous conditions associated with the inadvertent release of liquid ammonia to flare 12, while employees and contractors were performing work activities in the immediate area. The PHA did not address the consequences of liquid ammonia entering the flare system.

29 CFR  1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment:  On or about October 14, 2025, at the facility located at 701 E Martin St. Coffeyville, Kansas, in area 12 and area 11, the employer did not develop an effective written mechanical integrity procedure for process equipment that includes a set of specific actions and strategies detailing the scope, extent, methods and timing of specific inspection activities in order to determine the condition based on defined/expected damage of equipment, which includes: 1) Integrated knockout drum in flare 12. 2) PIC 0615 Bypass Valve.  Actions and strategies that were not addressed in the inadequate procedure include but are not limited to the following:  1) define the type(s) of inspection needed, (e.g. internal, external, on-stream, nondestructive);  2) identify the next inspection date for each inspection type;  3) describe the inspection methods; 4) describe the extent and locations of inspection and NDE at CMLs;  5) describe the requirements of any needed pressure test (e.g. type of test, test pressure, test temperature, and duration);  6) describe any required repairs if known or previously planned before the upcoming inspection.  7) describe the types of damage anticipated or experienced in the piping systems;  8) define the location of the expected damage;

29 CFR  1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment.  On or about October 14, 2025, in area 12 and area 11, employees engaged in nitrogen fertilizer production activities, located at 701 E Martin St. Coffeyville, Kansas, were exposed to asphyxiant hazards. The employer had not conducted inspections and tests to ensure the ongoing integrity of the process equipment. The employer did not conduct inspections and tests of the integrated knockout drum in flare 12 and the PIC 0615 Bypass Valve.
