Recreation Center, over pool, on or about October 13, 2025 - an employee engaged in scaffold erecting activities, was working third tier of the scaffold, when the unsecured Bil-Jax side bracket became dislodged from the scaffold. b) 2910 Trolly Road, Plymouth Meeting, PA 19462. Recreation Center, over pool, on or about October 13, 2025 - an employee engaged in scaffold erecting activities, was working third tier of the scaffold, where the unsecured Bil-Jax side bracket were secured to the scaffold by wire ties instead of the supplied pins recommended to use the side brackets. c)2910 Trolly Road, Plymouth Meeting, PA 19462. Recreation Center, over pool, on or about October 13, 2025 - an employee engaged in scaffold erecting activities, was working third tier of the scaffold, where unrated 5/8" plywood was used as decking.
