29 CFR  1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:   a) Aluminum Scrap Area - On or about October 15, 2025, an employee using an Oxygen and Acetylene torch to cut/remove rubber hoses from a gasoline V-Bottom Aluminum Fuel Tank that had not being cleaned thoroughly to ensure there were no flammable materials present, where subjected to heat might produce flammable vapors caused an explosion and fire to occur.  Employees used red ABC fire extinguishers and a yellow Class D fire extinguisher to put the fire out.  The employer did not provide an education program to familiarize the employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.

29 CFR  1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:   a) Aluminum Scrap Area - On or about October 15, 2025, an employee using an Oxygen and Acetylene torch to cut/remove rubber hoses from a gasoline V-Bottom Aluminum Fuel Tank that had not being cleaned thoroughly to ensure there were no flammable materials present, where subjected to heat might produce flammable vapors caused an explosion and fire to occur.  Employees used red ABC fire extinguishers and a yellow Class D fire extinguisher to put the fire out.  The employer did not provide an education program to familiarize the employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting upon initial employment or at least annually thereafter.
