29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Bone Stuffing Room - On or about October 14th, 2025, the employer did not ensure that employees implemented energy control steps prior to allowing employees to perform servicing/maintenance (removal of a jammed paddle) on the modified V-Mag 500/700 Stuffing Machine. Employees were subsequently exposed to the mechanical hazards of the rotating grinder screw and rotating grinder blade. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: (a) Bone Stuffing Room - On or about October 16th, 2025, employees were exposed to the amputation hazards created by the modified V-Mag 500/700 Stuffing Machine. The machine's hopper area was not adequately guarded in order to prevent employees from getting their hands and fingers into the rotating mechanical hazard of the machine's screw auger. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
