29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, or ungrounded electric power circuits that the employee could contact in the course of their work: a. On or about October 16, 2025, at the above addressed jobsite, an employee performing paint preparation activities from a JLG 460SJ Articulating Boom Lift was required to work within close proximity to energized electrical equipment, thereby exposing the employee to electrical hazards.

29 CFR 1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take. a. On or about October 16, 2025, at the above addressed jobsite, the employer did not ensure that the performance of the work would not bring the employee and/or equipment into physical or electrical contact with any part of the energized electrical equipment, nor did the employer post warning signs and advise the employee of the hazards and protective measures to be taken, thereby exposing the employee to electrical hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a. On or about October 16, 2025, at the above addressed jobsite, the employer did not train each employee performing work from a JLG 460SJ Articulating Boom Lift and a JLG 660SJ Articulating Boom Lift on the hazards associated with using and operating an aerial lift, nor did the employer train employees on the nature of any electrical hazards in the work area and correct procedures for dealing with electrical hazards.
