29 CFR 1910.27(b)(2)(ix): The employer did not ensure that the ropes of each rope descent system were effectively padded or otherwise protected, where they can contact edges of the building, anchorage, obstructions, or other surfaces, to prevent them from being cut or weakened.  Location: 560 West 168th Street, New York, NY.   On or about 10/19/2025,  a) The ropes used by employees to suspend their bosun's chairs were not protected against abrasion from making contact with sharp edges of the steel beam at the anchorage point on the roof.  b) The rope used by the deceased employee to suspend the bosun's chair was not protected against abrasion from making contact with the sharp edge of the metal bracket of the glass railing system.  The rope was cut and the employee fell approximately 60 feet during window washing operation.

29 CFR 1910.140(c)(20): The employer did not ensure that ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection were protected from being cut, abraded, melted, or otherwise damaged.  Location: 560 West 168th Street, New York, NY.   On or about 10/19/2025,  a) The ropes used for employees' lifelines were not protected against abrasion from making contact with sharp edges of the steel beam at the anchorage point on the roof.   b) The rope used for the deceased employee's lifelines was not protected against abrasion from making contact with the sharp edge of the metal bracket of the glass railing system. 29 CFR 1910.27(b)(1)(ii): The employer did not ensure that no employee used any anchorage before the employer obtained written information from the building owner that each anchorage met the requirements of paragraph (b)(1)(i) of this section.   Location: On the roof of 560 West 168th Street, New York, NY.   On or about 10/19/2025,  a) The employer did not obtain written information from the building owner to ensure each anchorage used by employees for their bosun's chairs met the load requirement.  Employees made their own determination on where to anchor their ropes based on their experience.  Mark Prince LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.27(b)(1)(ii)], which was contained in OSHA inspection number 1615967, citation number 1,  item number 1 and was affirmed as a final order on May 1, 2023, with respect to a workplace located at 189 Varick Street, New York, NY 10014.

29 CFR  1910.140(c)(12): The employer did not ensure that anchorages used to attach to personal fall protection equipment were independent of any anchorage used to suspend employees or platforms on which employees worked.      Location: On the roof of 560 West 168th Street, New York, NY.   On or about 10/19/2025,  a) Anchorages used to attach to personal fall protection equipment were not independent of anchorages used to suspend employees' work platforms. The ropes used for the employees' lifelines and the ropes used for the employees' boson's chairs were bundled together and tied to the same anchorages.  Mark Prince LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1910.140(c)(12)], which was contained in OSHA inspection number 1615967, citation number 1,  item number 2 and was affirmed as a final order on May 1, 2023, with respect to a workplace located at 189 Varick Street, New York, NY 10014.
