29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about October 23, 2025, at the worksite located at 721 East Murdock, Wichita, Kansas 67214, at the Straightening Area, employees engaged in metal straightening work were exposed to caught-in, crushed and/or amputation hazards in that the Dake Hydraulic Press Brake was not equipped with an adequate guard to prevent employees from entering the point of operation.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years: At the worksite located at 721 East Murdock, Wichita, Kansas 67214, employees engaged in material handling activities were exposed to struck-by hazards. Employees using the Toyota forklift(s), have not received a refresher training at least once every three years. An employee operated forklifts in the course of their daily work, while the employee's performance had not been evaluated since 11/19/2019.
