29 CFR 1910.22(a)(1):29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) On the Kamloops Barge moored in Lake Pend Oreille's Scenic Bay. On or about October 22, 2025, employees performing acoustic testing were exposed to trip and fall hazards by walking over power cords, mooring lines and floor protrusions present on the decks of both the Yellow and K Barge.

On or about October 22, 2025, an orange extension cord was laying in a puddle of water on the deck of the K-barge, connected to another extension cord that was not protected by waterproof cord connectors.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) On the Yellow and Kamloops Barges. On or about October 22, 2025, an extension cord was interconnected "daisy chained" to another extension cord used as the power supply to equipment on the Kamloops barge. (b) On the Yellow and Kamloops Barges. On or about October 22, 2025, an extension cord used in a wet environment was not listed and labeled for that use. An orange extension cord was laying in a puddle of water on the deck of the K-barge, connected to another extension cord that was not protected by waterproof cord connectors.
