(a) On or about October 21, 2025, the employer had not developed, implemented and utilized hazardous energy control program to ensure an effective hazardous energy control program was utilized by employees performing servicing and maintenance on equipment, and that employees not directly associated with maintenance operations were adequately informed of the maintenance and servicing occurring within their work areas.

This exposed employee's hands, arms and body system to the rotating, caught-in and crushing hazards, likely to result in serious crushing and amputation injuries when performing maintenance and servicing work.

29 CFR 1910.147(d): The application of energy control (lockout or tagout) actions were not done in the following sequence: (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished: (a) On or about October 21, 2025, and at times prior, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of hazardous energy sources were performed prior to employees removing a pan guard from the Mantissa sorting conveyor system, located at electrical disconnect Enclosure 4447, to evaluate a mechanical issue with the rollers, chain and sprockets. This exposed employee's hands, arms and body system to the rotating, caught-in and crushing hazards, likely to result in serious crushing and amputation injuries when performing maintenance and servicing work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: (i) Clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section; (ii) Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; (iii) Remove the lockout or tagout devices as specified in paragraph (e)(3) of this section; (iv) Energize and proceed with testing or positioning; (v) Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and/or maintenance. (a) On or about October 21, 2025, the sequence of actions was not followed as listed above when the employer reenergized the Mantissa sorting conveyor system, located at electrical disconnect Enclosure 4447, to visually evaluate and listen for mechanical issue associated with with the rollers, chain and sprockets. Employees were not removed to safe distance or other form of temporary barrier implemented to prevent exposure to the unguarded rollers, chain and sprockets when the conveyor belt was reenergized. This exposed employee's hands, arms and body system to the rotating caught-on, crushing and amputation hazards.
