29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Bay 1 - On or about October 21, 2025, an employee was hospitalized after falling approximately 37 inches when stepping down out of the back of a pickup truck (2020 Chevrolet Silverado K2 - VIN 1GC3YLE79LF211826) onto a liftgate (MAXON, Model ME2), in a vertical position that was not secured and stowed away. The employer reported the hospitalization to OSHA on October 24, 2025 outside of the 24 hour requirement.
