29 CFR 1926.600(a)(6)(i): For lines rated 50kV or below, a minimum of 10 feet was not maintained between the energized line and the equipment. On or about October 17, 2025, at the worksite located at E3739 Partridge Road, Luxemburg, Wisconsin; Employees were exposed to electric shock and electrocution hazards when the boom of the Schwing America Concrete Pump Truck (Model S36X 2025-5 120/80TC, Serial # 1020403) was operated less than 10 feet from the overhead powerlines energized to approximately 14,000 volts which led to an electrical flashover between the overhead lines and the boom of the concrete pump truck.
