29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system.  (a) On or about October 24, 2025, at the construction site located at 4800 Santa Monica, Whitefish Bay, WI 53217, the employer did not ensure employees engaged in roofing activities on a commercial apartment building had a fall protection system in place and were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately thirty five feet to the concrete surface below.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a) On or about October 24, 2025, and dates prior, the employer did not provide written documentation of  fall protection training for exposed employees.
