29 CFR  1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.   29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about November 27, 2025, the employer did not ensure employees engaged in preparing a roof for shingle installation at a residential  home located at 28 Lake Forest Drive, Stow OH 44236, which were at various heights of 10 - 25 feet above the ground were protected by a guardrail system, safety net system, or personal fall arrest system. The employees were not tied off to the D ring in the back of the fall protection harness.

29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about November 27, 2025, employees were exposed to a struck by hazard while working  beneath an active roofing crew, which were at various heights of 10 - 25 feet, at a residential home located at 28 Lake Forest Drive, Stow OH 44236, without wearing protective helmets.  BEA Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA  number 1828307, citation number 1, item number 1 and was affirmed as a final order on 7/7/2025, with respect to a workplace located at 3750  West 169th Street, Cleveland, OH 44111.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about November 27, 2025, at the location of a residential  home located at 28 Lake Forest Drive, Stow OH 44236: employees were exposed to a fall hazard of approximately 10 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface.  BEA Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA  number 1817410, citation number 1, item number 1 and was affirmed as a final order on 6/11/2025, with respect to a workplace located at 27030 Hilliard Blvd, Westlake, OH 44145.
