29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun:  On or about November 3, 2025, at a residential job site, located at 8525 Old Eight Rd., Northfield OH 44067, the employer failed to ensure that employees, who were operating a pneumatic nail gun while performing roofing activities, were wearing protective eyewear. Employees were exposed to flying debris and nails from the nail guns they were using to apply the new roof shingles.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about November 3, 2025, at a residential job site, located at 8525 Old Eight Rd., Northfield OH 44067, the employer failed to ensure employees, who were working on a multi-unit residential roof of approximately 20+feet, were protected from a fall from height hazard in that employees were not wearing a personal fall protection harness nor were there any other fall protections in place.

Employees were exposed to a fall from height hazard of 20+ feet.
