29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about October 27, 2025, the employer failed to ensure that, the Form Fill and Seal Machine, production line #6, was locked and tagged out prior to allowing employees to change the blue conveyor belt after cleaning the line. Employees are required to hand place and wrap the blue belt around the lines drive sprockets on the belt frame, which are electrically energized. Employees are exposed to amputation hazards due to being caught in the drive sprockets.
