29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On October 24, 2025, at 751 SE County Road 36, Johnson City, KS, employees were exposed to fall hazards of approximately 16 feet, in that they worked adjacent to an open hole that measured approximately 7x7 foot while placing milk chillers on an elevated platform.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Employees working from the metal platform, during Chiller installation, were exposed to an approximate 16' fall through unguarded openings in the platform without adequate instruction on the processes to follow and equipment necessary to keep them from falling through the platform openings.
