On or about October 9, 2025, in the Wastewater Department, employees cleaning sludge and taking samples from the city water well were standing on top of an unsecured grate that was placed above an open city water well that was 4 feet or more without protection.   b.   On or about October 9, 2025, in the Wastewater Department, employees accessed a steam valve by crossing over the outside of a guardrail and lowered themselves onto an unsecured grate that was placed above an open city water well that was 4 feet or more without protection.

29 CFR  1910.30(a)(3)(i): The employer did not train each employee on the nature of the fall hazards in the work area and how to recognize them:  a.  On or about October 9, 2025, in the Wastewater Department, employees stepped onto the outside of a guardrail attached to a catwalk and lowered themselves onto an unsecured grate that was placed above an open city water well to access the lower level to shut off a grease steam valve without training to recognize this specific fall hazard that was over 4 feet.     b.  On or about October 9, 2025, in the Wastewater Department, employees stepped onto the outside of a guardrail catwalk / platform and lowered themselves onto an unsecured grate that was placed above an open city water well to access the lower level to clean sludge from the city water well without training to recognize this specific fall hazard that was over 4 feet.  c.  On or about October 9, 2025, in the Wastewater Department, employees accessing FEP which is 40 feet high were trained on fall hazards but it was ineffective as test results did not show that employees understood the recognition of fall hazards.

On or about October 9, 2025, in the Wastewater Department, employees turned off a steam valve,  by accessing the outside of a second level guardrail platform, stepping onto an unsecured grate that was over the city water well, and stepped onto the city well wall  to access steps to the lower level were not provided a safe means of access and egress.  b. On or about October 9, 2025, in the Wastewater Department, employees cleaning sludge from the city water well, accessed the outside of a second level guardrail platform, and stepped onto an unsecured grate and stood on it to shovel sludge out of the city well were not provided a safe means of access and egress.    c. On or about October 9, 2025, in the Wastewater Department, employees working on the floor level, had to walk next to an EQ tank and hot boiling grease cooker and duck under pipes to obtain samples, check motors, turn on and off valves for steam and to add chemicals to treat water were not provided with a safe means of access and egress.
