29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level. A free fall may be more than 6 feet (1.8 m) provided the employer can demonstrate the manufacturer designed the system to allow a free fall of more than 6 feet and tested the system to ensure a maximum arresting force of 1,800 pounds (8 kN) is not exceeded. On or about November 6, 2025, at the Simpson Canyon Well #2043, an employee was using a personal fall arrest system anchored near the crown block of the derrick and the employer did not ensure that the personal fall arrest system was rigged so that the employee could neither free fall more than 6 feet nor contact a lower level.

29 CFR 1910.181(g)(1):The employer did not ensure that thorough inspection of all ropes in use was made at least once a month nor that a certification record which included the date of inspection, the signature of the person who performed the inspection, and an identifier for the ropes which were inspected was prepared and kept on file where readily available. The employer did not ensure that any deterioration, resulting in appreciable loss of original strength was be carefully observed and determination made as to whether further use of the rope would constitute a safety hazard.
