29 CFR  1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   (a)  On or about November 12, 2025, the employer did not ensure that each affected employee used appropriate eye or face protection, such as safety glasses with side shields and/or face shields, when exposed to eye or face hazards from flying particles while sawing, grinding and polishing aluminum parts that were made in the foundry.

29 CFR  1910.133(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.   (a)  On or about November 12, 2025, the employer did not ensure that each affected employee used appropriate eye protection, such as safety glasses with side shields, when exposed to eye or face hazards from flying particles while sawing, grinding and polishing aluminum parts that were made in the foundry.

29 CFR  1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.   (a)  On or about November 12, 2025, the employer did not ensure each affected employee who wore prescription lenses wore appropriate eye and face protection when cutting and grinding on aluminum parts.

29 CFR 1910.132(d)(2):  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  (a)  On or about November 12, 2025, the employer failed to verify through a written certification that a workplace hazard assessment had been performed for employees who cut and ground aluminum parts in Building 8, for employees who operated large saws to cut molded aluminum parts, and for foundry employees working near molten aluminum.
