29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  1910.147(c)(4)(ii)(B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  a) On or about October 2, 2025, the employer did not ensure that an employee troubleshooting the Cresswood Chipper Machine's (SN:1590606) conveyor system in WCH/Building #2 had Lockout/Tagout Energy Control Procedures that included specific procedural steps to identify the exact location to shut off power to the machine and how to locate, the electrical rating of the electrical energy, and how to identify the electrical main power source and its switch, exposing employees to risk of unexpected energization and amputation injuries.

29 CFR  1910.147(f)(1):Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed:  29 CFR  1910.147(f)(1)(v): Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and/or maintenance.   a) On or about October 2, 2025, the employer failed to ensure that an employee performing troubleshooting activities on a Cresswood Chipper Machine's conveyor system (SN:1590606) and gearbox (Make-Tigear) properly deenergized the electrical energy source and reapplied energy control measures prior to continuing work, and as a result an employee received an amputation to the right pinky finger.

Electric equipment may not be used unless the following markings have been placed on the equipment:  1910.303(e)(1)(i) The manufacturer's name, trademark, or other descriptive marking by which the organization responsible for the product may be identified; and  1910.303(e)(1)(ii) Other markings giving voltage, current, wattage, or other ratings as necessary.  a) On or about October 2, 2025, at WCH/Building #2, the employer failed to ensure that an employee operating an electrical control panel for the Crestwood chipper machine's conveyor system was provided with the required information. The electrical control panel lacked manufacturer identification and other markings necessary to indicate voltage, current, wattage, or other ratings as required.
