29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels shall have guardrail systems which comply with subpart M of this part -- Fall Protection:  a) Jobsite located at 623 North 55th Street, Philadelphia, PA.- The employer did not ensure that employees walking across a scaffold plank approximately 7 feet from the ground level had a guardrail system installed; thus exposing employees to fall hazards.  Condition was observed on or about November 14, 2025.

29 CFR  1926.20(b)(1):The employer did not create or maintain a safety and health program to include, but not limited to, identification and control of hazards, training and education, and work safety rules and procedures to enable employees exposed to fall hazards to recognize and avoid unsafe conditions at the workplace.   a) Jobsite located at 623 North 55th Street, Philadelphia, PA. - The employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during  jobsite foundation activities, such as but not limited to fall hazards.  Condition observed on or about November 14, 2025.  The Campos Enterprises, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1), which was contained in OSHA inspection number 1747212, citation number 1, item number 1 and was affirmed as a final order on 11/27/2024, with respect to a workplace located at 4324-44 Lancaster Ave, Philadelphia, PA. 19104.

29 CFR  1926.501(b)(5):Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:  a) Jobsite located at 623 North 55th Street, Philadelphia, PA. -The employer did not ensure employees installing concrete foundation walls for a mixed use building, were protected from fall hazards by the use of a fall protection system. Employees were working from the face of the formwork, approximately 9 feet above ground level without the use of fall protection.  Condition was observed on or about November 14, 2025.  The Campos Enterprises, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(5), which was contained in OSHA inspection number 1747212, citation number 1, item number 2 and was affirmed as a final order on 11/27/2024, with respect to a workplace located at 4324-44 Lancaster Ave, Philadelphia, PA. 19104.
