29 CFR 1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about September 19, 2025, during a construction remodel project at a residential home located at 3358 E. 132nd St., Cleveland, OH 44120, the employer did not provide adequate instruction to employees on recognizing and avoiding unsafe conditions associated with the second-story balcony during the waste removal phase. As a result, employees were exposed to fall hazards of approximately 11.8 feet.
