29 CFR 1910.22(a)(3):(3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. On or about November 18, 2025, Harvard Gardens Rehabilitation and Care Center, did not ensure that walking working surfaces between the washers and the dryers and in front of the sink in the laundry room were free of slip, trip and fall hazards. The walking area had 2 metal covers and 1 particle board cover that was loose and cracked covering a drainage cutout in the floor. All three covers protruded up from the floor presenting an uneven walking surface.
