On or about November 18, 2025, with respect to an establishment located at N909 Midway Road, Hortonville, Wisconsin, the Heli forklift (Model No.: CPYD25-M1H, Serial No.: 230253A4407) was equipped with fork extensions which were not previously approved by the manufacturer, and which affected the capacity and safe operation of the vehicle. The capacity / operation plate was not updated to adjust the lifting capacity, exposing employees to tip over hazards.
