29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycles:  a.) Associated Metal Forming Technologies, dba Associated Spring Corry; 1900 Department - On or about 10/2/25, and at times prior thereto, the light curtain guarding set-up on the  load cell tester was not designed to prevent the entry of hands into the machine's danger zone.  The light curtains did not activate as soon as the equipment became active. An amputation occurred when an employee reached into the machine to change out cups and the hydraulic cylinder activated.  NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
