29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about November 4, 2025, at the jobsite, employees were exposed to amputation hazards in that, the Ironwood upcut saw (Model Cut18L Serial 110728) rotating blade located under the cutting table was not properly guarded.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) On or about November 18, 2025, in the production area of the warehouse near the Ironwood upcut saw, employees were exposed to burn and asphyxiation hazards in that, a fire extinguisher was blocked with merchandise products, wood and carts.
