(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: On or about September 25, 2025, at a worksite located at 3001 Towpath Trail, Broadview Heights, Ohio, the employer did not ensure an employee working from a HVAC unit platform located above an office ceiling with an unprotected side or edge approximately 9 feet, 9 inches above a lower level (office floor) was protected from falling by a guardrail system, personal fall protections system, personal fall arrest, travel restraint, positioning system, or other type of fall protection device or system.

29 CFR 1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about September 26, 2025, the employer did not report within 24 hours the in-patient hospitalization injury of an employee who has hospitalized as the result of a work-related incident on or about September 25, 2025. The employer was notified the injury resulted in an in-patient hospitalization injury on or about September 25, 2025, and OSHA was not notified of the injury by the employer.
