29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) 801 E Bishop St, Bellefonte, PA - On or about November 14, 2025, two employees were working from the roof of a one story commercial structure, approximately 9 feet above grade, with no fall protection systems in use. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). a) 801 E Bishop St, Bellefonte, PA - On or about November 14, 2025, two employees were working from the roof of a one story commercial structure, utilizing an extension ladder that was at an angle that significantly exceeded the "one-quarter of the working length" . The extension ladder was nearly parallel to the pitch of the 12/12 roof. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
