29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. (a) On or about October 3, 2025, the employer failed to ensure that the Muffin Depositor was adequately guarded to prevent employee access to the point of operation, in that employees were able to place their hands inside the machine and activate it during operation. As a result, employees were exposed to amputation hazards from moving mechanical parts within the point of operation. (b) On or about November 19, 2025, the employer failed to ensure that the rear section of the Muffin Depositor was properly guarded to prevent employee contact with moving components, in that employees were exposed to air-activated cylinders that slid forward during dough depositing operations. As a result, employees were exposed to caught-in hazards from the unexpected movement of pneumatic components.

29 CFR 1910.1030(f)(2)(i):Hepatitis B vaccination shall be made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons. a) On or about November 19, 2025, the employer failed to offer the hepatitis B vaccination series to employees assigned to the United Dairy Farmers Bakery who had reasonably anticipated occupational exposure to blood or other potentially infectious materials. The employer did not provide vaccination offers to affected employees. As a result, employees were exposed to the hazard of infection following potential contact with blood or other potentially infectious materials.

29 CFR 1910.1030(g)(2)(i):The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program. a) On or about November 19, 2025, the employer failed to ensure that employees received training on bloodborne pathogens prior to performing cleaning of blood, in that an employee was instructed to clean a contaminated machine following an amputation incident at the United Dairy Farmers Bakery. As a result, employees were exposed to hazards associated with bloodborne pathogens due to lack of required training.
