29 CFR 1910.23(c)(13): The employer did not ensure that ladders are not placed on boxes, barrels, or other unstable bases to obtain additional height. Builders Concrete East, LLC, 79 Boston Post Road, Willimantic, CT, Erie strayer concrete batch plant: On or about November 18, 2025, employees were permitted to place a Gorilla Ladders portable ladder on the bed of a work truck to access the batch plant chute approximately 14 feet above the ground while performing welding/maintenance activities. The truck bed constituted an unstable base as it could move; placing the ladder on it to gain additional height is prohibited. This condition exposed employees to a fall hazard of approximately 14 feet to a lower level.

The employer did not establish and implement a program consisting of energy control procedures, employee training, and periodic inspections: Builders Concrete East, LLC, 79 Boston Post Road, Willimantic, CT: On or about November 18, 2025, an employee was caught between the drip pan and the chute while performing welding/maintenance on an energized Erie Strayer concrete batch plant without machine?specific lockout/tagout procedures covering multiple energy sources, training, and/or annual periodic inspections.
