Builders Concrete East, LLC, 79  Boston Post Road, Willimantic, CT, Erie strayer concrete batch plant:              On or about November 18, 2025, employees were permitted to place a Gorilla Ladders portable ladder on the bed of a work truck to access the batch plant chute approximately 14 feet above the ground while performing welding/maintenance activities. The truck bed constituted an unstable base as it could move; placing the ladder on it to gain additional height is prohibited. This condition exposed employees to a fall hazard of approximately 14 feet to a lower level.
