29 CFR  1910.30(a)(3): Fall protection. The employer must train each employee in at least the following topics:   (i) The nature of the fall hazards in the work area and how to recognize them; (ii)The procedures to be followed to minimize those hazards; (iii)  The correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses; and (iv) The correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.  a) On or about November 11, 2025, the employer did not ensure that employees were adequately trained when exposed to a 10 feet fall from the top of truck cab while removing CNG cylinders. Employees were not trained on the topics addressed in 1910.30(a)(3)(i) through (iv) in that employees were anchored to an employer made personnel cage attached to the forks of a forklift while working on the roof of the cab, employees were not anchored above or on the truck, portions of the guardrails on the cage were not rigid, and employees were not trained on how to inspect personal fall protection equipment as the lanyard was damaged.

29 CFR  1910.140(c)(18): General requirements. The employer must ensure that personal fall protection systems meet the following requirements. Additional requirements for personal fall arrest systems and positioning systems are contained in paragraphs (d) and (e) of this section, respectively. Personal fall protection systems must be inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components must be removed from service.  a) On or about November 11, 2025, the employer did not ensure that personal fall protection systems worn by employees were inspected before use in that a damaged and worn lanyard worn with a fall protection harness was used to reach the top of a truck cab that was 10 feet high.
