29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) At the site located at 10500 Buckingham Rd, Fort Myers, Florida: On or about 10/17/2025, the employer exposed employees to caught-in hazards, in that, employees were conducting maintenance on the Plattco (a.k.a. double dump) at the bottom of the #1-SDA Hopper without having a lockout/tagout procedure in place for the specific machine prior to maintenance being done.

Double Dump) at the bottom of #1-SDA Hopper were not guarded to prevent employees from getting their hands caught in the cylinders while they were running.
