Carpenter shop: On or about November 19, 2025, and at times prior to, employees utilized Safety Speed Manufacturing Vertical Panel Saw (Model: 6400, Serial no: 75282) to cut big sheets of wood into smaller pieces without cleaning the immediate working area. This condition potentially exposed employees to slip trip fall hazards and/or wood dust, debris fire hazard(s).

29 CFR  1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.   Carpenter shop: On or about November 19, 2025, and at times prior to, employees cut chemically treated wood pieces using the System Safety Manufacturing Vertical Panel Saw (Model: 6400, Serial no: 75282) donning a surgical mask. The employer did not ensure a hazard assessment was conducted to determine the presence of respiratory hazards and/or if a surgical mask was sufficient for employee protection. These conditions potentially exposed employees to respiratory hazard(s).

Carpenter shop: On or about November 19, 2025, and at times prior to, employees conducted wood repair work using Titebond II Premium Wood Glue without having a label to identify and provide safety information.
