Instance (a): On or about October 23, 2025, the employer did not document and utilize specific lockout/tagout procedures for the header or wire drawer machine on line AT-810?2. An employee was performing setup on the wire and header machine without a written procedure for energy isolation. Instance (b): On or about November 13, 2025, the employer did not document and utilize specific lockout/tagout procedures for RT Cell #1. Employees were performing maintenance while replacing the motor without a written procedure for energy isolation.

1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: 29 CFR 1910.147(f)(1)(i): Clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section. On or about October 23, 2025, the employer did not ensure the required sequence was followed when temporarily energizing the wire drawer machine on line AT-810?2 for positioning of the wire. Employees are exposed to amputation injuries while remaining near the danger zone between the wire and the capstan during machine use. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met. On or about October 23, 2025, and continuing through November 19, 2025, the employer had no written program describing how chemical hazards would be communicated to employees. Employees in the heading department were exposed to various chemicals, including, but not limited to, LPA-142 Solvent, RENEP KN 68 NA lube oil, Mean Green cleaner, and CLC Draw SKB cleaner without a documented system to ensure proper identification, communication, and control of those hazards.
