29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Roof - On or about November 20, 2025, employees were exposed to eye hazards when working on the roof using tools including, but not limited to, nail gun, shingle scraper tools, and a blower to rip up and remove the old shingles, along with installing new shingles on the roof without wearing eye protection.

Employees were not provided with lifelines, anchor points, harnesses, and lanyards.
