29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, shall be 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: On or about November 10, 2025, and at times prior to, employees of Columbia Curb & Gutter Co. were installing concrete forms on a bridge at 13816 MO-8 in Potosi, Missouri. The employer was utilizing a guardrail system with a top rail height at approximately 59 inches above the walkway surface exposing employees to an approximate 25-foot fall hazard to the railroad track below.

29 CFR 1926.502(b)(2)(iv): Other structural members (such as additional midrails and architectural panels were not installed such that there are no openings in the guardrail system that are more than 19 inches (.5m) wide: On or about November 10, 2025, and at times prior to, employees of Columbia Curb & Gutter Co., were installing concrete forms on a bridge located at 13816 MO-8 in Potosi, Missouri. The employer utilized a guardrail system where openings between the toprail, midrail, and vertical posts were greater than 19 inches wide exposing employees to fall hazards of 20 feet to the railroad tracks below.

The employer did not report the injury to the Occupational Safety and Health Administration (OSHA) within the required twenty-four (24) hours.
