29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about November 20, 2025, the employer did not ensure that authorized employees received necessary training in the recognition of hazardous energy sources, the types and magnitude of energy present in the workplace, and the methods and means required for energy isolation prior to performing blade changes on the RUN-A-GADE High Volume Band Saw system. As a result of this lack of training, employees were exposed to several hazards during the blade changes, including unexpected machine startups, contact with moving parts, and electrical hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about October 10, 2025, the employer did not ensure adequate machine guarding for the RUN-A-GADE dismantling saw. The guarding system did not prevent access to the exposed rotating band saw blade at the point of operation while the machine was in operation, allowing operators to place their hands within the danger zone. This condition exposed employees to laceration and amputation hazards during pallet dismantling operations.
