29 CFR  1910.134(c)(2)(ii):  In addition, the employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user.   a.  Tru Kut Plasma Table:  On or about November18, 2025, employees wearing half mask negative pressure respirators were exposed to respiratory stressors associated with being allowed to wear the Miller LPR-100 while working on the plasma table.  Employees have not been medically cleared to wear the respirator and have not been trained on the cleaning, storage, and maintenance of the respirator.    Documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR  1910.134(e)(1): General. The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.  a.  Tru Kut Plasma Table:  On or about November18, 2025, employees wearing half mask negative pressure respirators were exposed to respiratory stressors associated with being allowed to voluntarily wear the Miller LPR-100 while working on the plasma table.  The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.134(h)(1): Cleaning and disinfecting. The employer shall provide each respirator user with a respirator that is clean, sanitary, and in good working order. The employer shall ensure that respirators are cleaned and disinfected using the procedures in Appendix B-2 of this section, or procedures recommended by the respirator manufacturer, provided that such procedures are of equivalent effectiveness. The respirators shall be cleaned and disinfected at the following intervals:  a.  Tru Kut Plasma Table:  On or about November18, 2025, employees wearing half mask negative pressure respirators were exposed to respiratory stressors associated with being allowed to wear the Miller LPR-100 while working on the plasma table.  The employer did not ensure that respirators are cleaned and disinfected.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.1026(d)(2)(i): The employer shall perform initial monitoring to determine the 8-hour TWA exposure for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area. Where an employer does representative sampling instead of sampling all employees in order to meet this requirement, the employer shall sample the employee(s) expected to have the highest chromium (VI) exposures.  a. Tru Kut Plasma Table:  On or about March 4, 2026, employees conducting plasma cutting operations of hot rolled plates/slabs were not evaluated for initial exposure to hexavalent chromium. The employer did not perform initial monitoring to determine the 8 hour TWA exposure for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area. Where an employer does representative sampling instead of sampling all employees in order to meet this requirement, the employer shall sample the employee(s) expected to have the highest chromium (VI) exposures.  b. Welding Shop:  On or about March 4, 2026, employees welding in the welding shop were not evaluated for initial exposure to hexavalent chromium. The employer did not perform initial monitoring to determine the 8 hour TWA exposure for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area. Where an employer does representative sampling instead of sampling all employees in order to meet this requirement, the employer shall sample the employee(s) expected to have the highest chromium (VI) exposures.  This item was corrected during inspection.

29 CFR  1910.1026(l)(1)(iii): Employers shall include chromium (VI) in the hazard communication program established to comply with the HCS (§ 1910.1200). Employers shall ensure that each employee has access to labels on containers of chromium (VI) and to safety data sheets, and is trained in accordance with the requirements of HCS and paragraph (l)(2) of this section.   a. Tru Kut Plasma Table:  On or about March 4, 2026, employees were exposed to respiratory health hazards such as but not limited to hexavalent chromium.  The employer did not include chromium (VI) in the hazard communication program, and train employees in accordance with the requirements of the Hazard Communication Standard and 29 CFR 1910.1026.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
