29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) On or about November 20, 2025, at the Back Shop/Shop 2 exit door, the employer did not ensure that the exit route door could be opened from the inside at all times without keys, tools, or special knowledge. The door was secured with a latch requiring a key or unfamiliar operation, thereby delaying or preventing emergency egress. Employees were exposed to fire and smoke inhalation hazards due to the potential inability to exit promptly during an emergency.

29 CFR 1910.27(a): The employer did not ensure that scaffolds used in general industry meet the requirements in 29 CFR part 1926, subpart L (Scaffolds). a) On or about November 20, 2025, and at times prior, at Shop 1, the yellow scaffold was not inspected by a competent person before each work shift. The scaffold had visible structural defects, improper assembly, and missing components. b) On or about November 20, 2025, and at times prior, at Shop 2, the blue and yellow scaffolds were not inspected by a competent person before each work shift. These scaffolds had visible structural defects, improper assembly, and missing components.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) On or about November 20, 2025, at Shop 1, the portable fire extinguisher mounted on the pillar to the left of the exit door was not clearly identified or marked. The lack of signage or visual indicators could delay employees in locating the extinguisher during a fire emergency, increasing the risk of fire spread and injury. b) On or about November 20, 2025, at Shop 2, the portable fire extinguisher located to the right of the exit door was not clearly identified or marked. This condition could hinder prompt access during a fire emergency, exposing employees to burn and smoke inhalation hazards.
