29 CFR  1910.151(b):First aid supplies approved by a consulting physician were not readily available:  a)  Throughout Building:  On or about November 20, 2025, employers did not secure expired medications in posted first aid lock boxes throughout the facility, exposing employees to possible allergic reactions.  b) Main Hallway near bathrooms:  On or about 20 November, employers did not secure expired electrodes from a wall mounted Automatic External Defibrillator (AED) , exposing employees to burns and shock hazards.

Employees engaged in material handing activities using the TMG12 Clark forklift, serial number, TMG248-0564-7498FB were exposed to struck-by hazards in that comprehensive forklift training was not provided prior to permitting forklift operation.
