29 CFR  1910.37(a)(3):  Exit route(s) were not kept free and unobstructed:  Palla Mill Conex Box:  On, or about, October 27, 2025, the single entrance/egress door was not accessible at all times.  An electrical component failed in the 480 volt motor control center enclosure, which caused an explosion and smoke.  An employee had to pass the area to use the only exit door that was provided for the workspace.

29 CFR  1910.303(g)(1)(i)(A):  For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:  Palla Mill Conex Box:  On, or about, November 7, 2025, a work table was located on the wall opposite from the Allen Bradely 480 volt machine control enclosure.  The table was measured to be 37 inches from the face of the enclosure.

29 CFR  1910.303(g)(1)(i)(B):  For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater :  Palla Mill Conex Box:  On, or about, November 7, 2025, adequate working space was not provided for the 480 volt Eaton fused disconnect that was located between the entrance door and the transformer.
