Supply Storage Room: On or about November 21, 2025, and at times prior to, the  Defiant 6-Outlet Relocatable power taps (power strips) were series-connected (daisy-chained) to extension cords or other power strips. These conditions potentially exposed employees to electrical and fire hazards.
